Service Coordination

The Service Coordinator will provide you / your family member with assistance with daily affairs including the following:

Helping to secure a residence in the community that meets the participant’s welfare and safely needs.

Gathering information to help the participant develop an individualized plan for the most independent lifestyle possible.

Assisting the participant to carry out this plan within the community.

Providing support and encouragement to the participant to increase his/her ability to problem solve and to be in control of everyday life situations.

Assuring that the participant is as independent as possible while maintaining a safe environment.

Assuring that there is adequate coordination, appropriate communication and maximum cooperation between all sources of support and services for the participant.

The Service Coordinator’s ultimate responsibility is to assure that the plan created by the participant is appropriately implemented and followed through to completion.
